Nursing assistant license renewal and issuance method
In order to become a nursing assistant, you must complete the curriculum prescribed by the Ordinance of the Ministry of Health and Welfare in accordance with Article 80 of the Medical Act, pass the national examination for nursing assistants, and receive qualifications from the Minister of Health and Welfare. 1. Certificate of graduation from the last school or certificate of academic achievement (including certificate of scheduled operation)
2. Certificate of Completion of the Nursing Assistant Training Course in Attached Form No. 2 issued by the head of the National Public Assistant Nursing Assistant Training Center and the head of the academy that operates the nursing assistant training course
3. Nursing Assistant Medical Institution Practice Completion Certificate in Attached Form No. 3 issued by the head of the internship medical institution
4. A doctor’s diagnosis proving that you do not fall under Article 8 Subparagraphs 1 and 2 of the Medical Act
5. Foreign university or junior college graduation certificate and copy of foreign nurse’s license
6. Certificate of completion of foreign nursing assistant training course and copy of foreign nursing assistant license
7. A color photo of the upper body taken without a background within 6 months from the date of submission. The size is 3 x 3.5 cm x 4.5 cm x 3 photos.
You can get a reissue if you correct the registration details or if your license is worn out and is no longer usable. Two photos are required, and an application must be made to the Minister of Health and Welfare or the governor of a city or city for the reissuance of a license or the correction of certificates or descriptions of a certificate.
In the case of reissuance of license or application for certification, fees must be paid with revenue stamps, revenue stamps, or electronic money or electronic payment using the information and communication network.
